General Data Protection Regulations
You will be asked to complete an online medical disclaimer and terms and conditions form.
In which you will be asked to provide your contact details. If you consent to sharing these with Dan Kyte
will only be contacted by Dan directly, for the following purposes:
a) to arrange a class with you upon your request.
b) if Dan is unable to teach a class or for class updates due to unforeseen circumstances.
c) to find your location.
Dan collects and holds emergency contact details for you. These will only be used in the event of an emergency should Dan need to contact your next of kin or provide their contact details to the emergency services.
If at any stage you do not want Dan to contact you anymore tell Dan directly. You can also ask to see, edit and/or destroy your health questionnaire and any information held, relating to you, at anytime.
Your disclaimer form will be securely stored for up to 7 years unless you request otherwise.
All photo's or videos taken during your session will be deleted once you have confirmed you have received and saved your mobility/yoga plan.
No bank details will ever be stored and Dan would only ask you for these if you should request a refund via a bank transfer. Alternatively, you can request a cash refund.
All data held will be kept strictly confidential between Dan and youself. Your data will not be shared with any third parties. Dan Kyte will only use your data for the purposes above and in agreement with you.
